(Alexandria, MN)  Horizon Public Health (HPH) oversees five counties in West-Central Minnesota.  As it does every five years HPH looks to get a more accurate account of what the needs and desires are within the communities they serve. 

More specifically Horizon Public Health has launched a Community Health Survey to better understand how the environment in which people live, work, play and age shapes overall health.

Information from the survey will be analyzed to help shape future programs and healthy living initiatives in the counties that Horizon Public Health serves. This data will become part of the 2022 Horizon Public Health Community Needs Assessment.

Horizon Public Health is asking you to please take this short survey, which will take less than 10 minutes to help our agency. They also ask that you please forward this to others who live in Douglas, Grant, Pope, Stevens and Traverse counties for them to complete.

The deadline to fill out the survey is November 29th.
